Presentation
51 y/o presenting with seizures.
Image Findings
High signal on T2- and low signal on T1-weighted images in the left parietal white matter and to a lesser extent, in the right parietal white matter. The cortex is spared and no enhancement is seen.
Differential Diagnosis
- Progressive multifocal leukoencephalopathy
- Lymphoma
- Low grade glioma
- Other viral encephalitis
Discussion
HIV encephalitis is the major cause of brain infections in patients with HIV. The lack of enhancement and the bilateral nature of this patient's findings goes against tumors and opportunistic fungal infections.
