63 y/o with acute knee pain after sitting on his knees.
A well defined, oval fluid collection appearing bright on the T2-weighted images is seen in the popliteal fossa. Also note the linear area of increases signal is noted in the posterior horn of the medial meniscus, extending to the articular surface of the meniscus, in keeping with a grade 3 tear.
Baker's or Popliteal cysts occur at higher frequency with degenerative joint disease of the knee and with meniscal tears.
