53 y/o male with squamous cell carcinoma of the face.


Image Findings and Discussion

An  enhancing soft tissue mass is seen in the left infraorbital foramen, extending along the course of V2 back to the cavernous sinus. This is compatable with perineural spread of the patients squamous cell carcinoma. Fat suppression may help the conspicuity of subtle enhancement, especially in the optic nerve. At high field strengths, susceptibility artifacts from the tissue-air interface at the margins of the maxillary sinus may actually be a hinderance if fat suppression is used to look at the V2 nerve.
